Job Responsibilities

• Provide comprehensive palliative care, including symptom management and supportive treatment planning
• Lead goals-of-care discussions and advance care planning with patients and families
• Coordinate care across multidisciplinary teams, including primary and specialty providers
• Maintain accurate, timely, and compliant medical documentation
• Participate in quality improvement initiatives and patient-centered care programs

Qualifications

• MD or DO degree
• Board Certified or Board Eligible in Hospice & Palliative Medicine or Palliative Care
• Active Oregon medical license or eligibility to obtain one
• Experience in palliative care, hospice, or supportive care settings preferred
• Commitment to compassionate, patient-centered, culturally responsive care
