Job Title: Care Manager I, Adult (Nashville, TN) *$1,500 sign‑on bonus opportunity. Schedule: Monday–Friday, 8:00am – 4:30pm. Salary: $41,000 (base pay) with additional incentive for language skills and behavioral health experience. Mileage reimbursement available. Job Summary Provide comprehensive care‑management services to consumers with severe and persistent mental illness (SPMI) and their supports. Responsibilities Conduct continual assessment of consumers’ strengths, needs, supports, and goals. Meet with consumers and their supports in the community, home, school, court, and appointments in office or other community locations. Collaboratively complete comprehensive and individualized care plans, crisis plans, PRAPARE screening, CSSRS, and DLA/CANS assessment tools within 30days of admission and every 6months thereafter. Assist with referral and linkage to internal and community resources to ensure consumers receive needed supports. Provide side‑by‑side support to consumers in addressing barriers and learning new skills, and advocate for consumers as necessary. Assist consumers with attending appointments (Clinic, Therapy, IHC/PCP) by providing reminders, addressing barriers, and providing transportation when needed. Educate consumers and supports about gaps in care, develop action plans, and manage health proactively. Educate consumers and supports about appropriate use of Emergency Department, inpatient care, PCP, and urgent care. Provide supportive interventions when a consumer utilizes ED/IP, following agency guidelines. Ensure daily psychiatric, medical, and environmental needs are planned for, coordinated, and documented. Ensure timely, efficient transitions of consumers within the agency and community as needed. Meet productivity standards as indicated in the Policies and Procedures. Attend monthly supervision meetings with the Care Management Supervisor, coming prepared with caseload information, questions, and solutions. Attend daily in‑person team meetings, coming prepared with caseload information, questions, and solutions. Actively participate in collaborative problem solving and team building. Collaborate with team members, agency employees, and community partners to promote consumer success. Follow up with plans from supervision and team meetings within agreed deadlines. Respond to urgent/crisis consumer needs using supervisor, crisis services, and clinic staff guidance. Develop and maintain positive relationships with consumers’ supports and community contacts. Requirements Bachelor’s Degree in health‑related field of counseling, psychology, social work, or other behavioral health field. If holding a bachelor’s degree in criminal justice, must have fifteen college‑level semester hours of coursework in behavioral health. Valid Tennessee Driver’s License.
