Licensed Practical Nurse (LPN) – Home Care

Location: West Philadelphia, PA Schedule: Contract, 40 hours weekly, 8am – 4:30pm

Position Overview

The Licensed Practical Nurse delivers high-quality, patient-centered nursing care to clients in their homes. Working independently and as part of an interdisciplinary team, the LPN supports individuals with acute, chronic, or post-operative needs, promoting health, independence, and quality of life.

Key Responsibilities

Provide direct nursing care in patients' homes in accordance with individualized care plans
Administer medications, injections, and treatments as prescribed
Perform wound care, dressing changes, and monitor healing progress
Assess patient condition and report changes to the RN or care team
Assist with activities of daily living (ADLs) such as bathing, grooming, and mobility support
Educate patients and families on disease management, medications, and home safety
Maintain accurate and timely clinical documentation
Coordinate care with RNs, physicians, and other healthcare professionals
Ensure infection prevention and control practices are followed in the home setting
Travel between patient homes within the community

Qualifications

Active LPN license in Pennsylvania
Current BLS (Basic Life Support) certification
Valid driver's license and reliable vehicle (required for community visits)
Minimum 2 years of nursing experience preferred
Previous home care or community health experience is an asset
Wound care experience
