Job Description
The Care Coordinator plays a key role in coordinating participant care, supporting families and caregivers, conducting assessments, developing individualized care plans, and connecting participants with community resources. Working closely with nursing, rehabilitation, activities, and social service staff, the Care Coordinator helps ensure participants receive comprehensive, person-centered care.
