Are you a Hospital Liaison, Clinical Liaison, Healthcare Marketer, Admissions Liaison, or Business Development professional who loves being in the field, building relationships and turning opportunities into results?

We are growing our Ohio market and looking for a high-energy, relationship-driven Patient Care Liaison to help connect patients with exceptional skilled nursing and post-acute care.

This is not a desk job and it isn't about dropping off brochures.

You'll be out in the market developing hospital and healthcare partnerships, meeting with case managers and discharge planners, identifying referral opportunities, helping patients navigate the transition to post-acute care, and directly impacting admissions and census growth.

What You'll Do

• Develop and grow hospital and community referral relationships

• Generate qualified referrals for our Skilled Nursing Facilities

• Conduct patient and family bedside visits when appropriate

• Respond quickly to referral opportunities and help move patients from referral to admission

• Educate referral partners on our clinical capabilities and post-acute services

• Identify market opportunities, competitor activity and barriers to placement

• Partner with facility leadership and clinical teams to improve referral conversion

• Track referral activity, admissions, conversion and market growth

• Maintain a strong, consistent presence throughout your assigned territory

• Help build strategies that drive Medicare, skilled admissions and overall census

We're Looking for Someone Who Is

You'll be a great fit if you have:

• 2+ years of experience in healthcare sales, hospital liaison, clinical liaison, skilled nursing marketing, admissions, home health, hospice, senior care or post-acute business development

• Existing knowledge of the hospital discharge and post-acute referral process

• Experience building relationships with case managers, social workers, discharge planners and physicians

• A proven ability to generate referrals, convert opportunities and drive admissions

• Strong communication, organization and follow-through

• The ability to work independently and manage a field-based territory

• A solutions-first mindset and the urgency to get things done
