Living Well Disability Services delivers exceptional services that transform the lives of people impacted by intellectual, developmental, and physical disabilities.

We provide services in people's own homes, their family homes, and 36 group homes throughout a 10-county region. We support people with a wide range of disabilities and support needs, including cerebral palsy, Down syndrome, multiple sclerosis, autism, and brain injury, which may be accompanied by complex health conditions.

Through innovation in service delivery, accessibility, wellness, and technology, we help people pursue their goals, build independence, and live well .

• Lead and develop a diverse team of Direct Support Professionals, CNAs, Assistant Program Managers, and Advanced Assistant Program Managers through effective hiring, training, supervision, mentoring, and performance management.
• Develop and implement individualized services that reflect the goals and preferences of the people we serve and the direction of their legal representatives and interdisciplinary teams.
• Monitor service quality and evaluate outcomes to ensure services meet individual needs, agency standards, and regulatory requirements.
• Manage service locations to achieve operational, financial, staffing, and human resource goals.
• Build strong relationships through effective communication with employees, people receiving services, families, legal representatives, coworkers, and interdisciplinary team members.
• Manage purchasing and budgets to ensure necessary supplies are available while meeting established budget targets.
• Develop staff schedules that ensure adequate coverage and meet the needs of the people we serve.
• Maintain regulatory compliance by completing required documentation, reports, and communications related to licensing and service requirements.
• Maintain required certifications and training , including CPR, First Aid, Medication Administration, and/or Mandt, as applicable.
• Perform other duties and responsibilities as assigned to support the mission and success of Living Well Disability Services.

• Passion, willingness, and courage to make a difference in the lives of people with disabilities.
• One of the following:
  • Four-year post-secondary degree and QIDP credential; or
  • Bachelor's degree in a human services-related field and one year of full-time experience providing direct care services to people with disabilities or adults age 65 and older; or
  • Associate degree in a human services-related field and four years of full-time experience providing direct care services.
• Valid driver's license.
• Successful completion of a background study through the Minnesota Department of Human Services.
