In this role, you will manage a dynamic caseload, assess patient needs, and collaborate with physicians, care teams, and families to develop and execute effective, cost-conscious care plans. You’ll play a key role in streamlining care delivery, removing barriers, and ensuring smooth transitions across the continuum of care.This position is ideal for a proactive professional who thrives in a fast-paced environment, excels at communication, and is passionate about improving outcomes. You’ll advocate for patients while partnering with payers and providers, oversee utilization and length of stay, and drive safe, timely discharges that reduce readmissions.
