Case Manager Location: 355 Grand Street, Jersey City, NJ 07302 Facility: Jersey City Medical Center Department: RW Part A - EHA Job Type: Full‑Time, Day shift Ref #: 0000255645 Pay: $28.63 - $32.93 per hour Pay Transparency: The above reflects the anticipated hourly wage range for this position if hired to work in New Jersey. The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience. Job Overview Responsible for providing comprehensive medical case management, health, and psychosocial education to CCC patients, families and significant others. Qualifications Bachelor's Degree in Social Work or a related social work field. Demonstrated knowledge of case management standards. Ability to function as a member of an interdisciplinary team. Master's Degree in a mental health field that leads to licensure - strongly preferred. Experience and knowledge of HIV, entitlement programs, and community resources - preferred. Essential Functions Conduct risk reduction, adherence counseling and provide referrals to patients with the goal of preventing HIV transmission. Provide counseling and information regarding advance directives and assist patients in completing forms. Refer and link patients to services that promote positive health outcomes, treatment adherence, and greater self-sufficiency. Maintain up‑to‑date records of all patient encounters and necessary documentation in the chart. Monitor and complete timely documentation to ensure follow‑up of all referrals. Provide access to emergency financial assistance (e.g., food vouchers, utility payment assistance, transportation) as needed. Provide supportive counseling as needed and refer appropriate patients to the mental health counselor. Maintain HIPAA guidelines and confidentiality when interacting with patients, families, personnel, and the public. Respond in a timely manner to verbal and written requests from patients, interdisciplinary team members, and administration, medical and public agencies. Attend scheduled medical case management trainings and meetings. Comply with unit and departmental scheduling guidelines. Promote and foster a positive work environment. Perform other duties as assigned.
