Case Manager - OPD

Overview Mount Carmel Guild Behavioral Health System, a CARF accredited division of Catholic Charities of the Archdiocese of Newark, is a leading provider of mental health and addiction services in Northern New Jersey. We are currently seeking a Full Time Case Manager for our Outpatient Department in Jersey City, NJ. Our Outpatient Program uses a multi-disciplinary approach; emphasis is placed on individualized assessment, goal-oriented treatment planning, periodic case review, and continuity of care for all persons served. Responsibilities Function as part of OPD team Interfaces with OPD psychiatrist Assists staff psychiatrist with scribing clinical assessments Completes treatment plans and maintains staff psychiatrist caseload of clients Completes documentation according to agency and DMHAS standards Qualifications Requires bachelor\'s degree in a human services field Additional Information Salary: $40,000 - $45,000 Visit our website: CCAN is an equal opportunity employer.
