WI-SSM Health St. Mary's Hospital - Janesville/n /n /n /n Worker Type: PRN/n /n /n Job Summary: Evaluates and treats patients with physical, emotional, cognitive and/or psychosocial deficits./n /n /n Job Responsibilities and Requirements: /n PRIMARY RESPONSIBILITIES /n /n - Assesses patient 's ability to safely and effectively perform independent living skills. Designs the appropriate plan of care, including type and frequency of treatment. /n - Implements the treatment plan using appropriate modalities. Documents findings of patient progress. Monitors and modifies the plan of care as needed. Seeks consultation as necessary. /n - Educates the patient and family/caregiver about patient deficits. Plans and/or assists with patient discharge from therapy services and provides information regarding appropriate selection/use of adaptive equipment and support programs. /n - Oversees the care provided by designated individuals according to established regulations and practice standards. /n - Works in a constant state of alertness and safe manner. /n - Performs other duties as assigned. /n /n /n EDUCATION /n /n - Bachelor's, Master's or Doctorate degree in Occupational Therapy, or related area as required to obtain licensure in state of practice /n /n /n EXPERIENCE /n /n - No experience required /n /n /n PHYSICAL REQUIREMENTS /n /n - Constant standing. /n - Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs. /n - Frequent use of hearing and speech to share information through oral communication. /n - Frequent use of hearing to distinguish body sounds and/or hear alarms, malfunctioning machinery, etc. /n - Frequent use of vision for distances near (20 inches or less) and far (20 feet or more). /n - Frequent walking, reaching, gripping and keyboard use/data entry. /n - Occasional bending, stooping, climbing, kneeling, sitting, squatting, twisting and repetitive foot/leg and hand/arm movements. /n - Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs. /n - Occasional lifting/moving patients. /n - Occasional use of vision to judge distances and spatial relationships and to identify and distinguish colors. /n - Occasional driving. /n - Rare crawling. /n /n /n REQUIRED PROFESSIONAL LICENSE AND/OR CERTIFICATIONS /n /n Resuscitation Certification Requirement: Basic Life Support Healthcare Provider (BLS HCP) required within 90 day of employment.
