Job Description Job Description Hospice Social Worker, MSW PRN/Per Diem Trellis Supportive Care is seeking a PRN/Per Diem Hospice Homecare Social Worker (MSW) to provide individualized hospice services in patients homes and facilities in Stokes County and surroundings areas. Trellis Supportive Care has made it our mission to provide compassionate care for individuals living with a life-limiting illness and their families, through quality medical, emotional, spiritual, and social support wherever care is needed. Our staff has grown to over 350 specially trained professionals who work as a team to help patients live fully and comfortably, with hope — even during life’s most challenging time. Schedule: PRN/Per Diem to make social work visits to transitional and end of life patients and their families on Saturdays & Sundays Availability for week day training needed in the beginning Start and end your day from your home - flexibility to manage your schedule Serving Forsyth County and other surrounding Counties - office located in Winston Salem, NC Exceptional mileage reimbursement Excellent PRN pay incentives Position Requirements: Master’s degree in Social Work required. Knowledge of the community’s social, financial, and health resources. Commitment to and understanding of the Hospice philosophy. Position Duties and Responsibilities: Provide individualized Hospice services to patients and families in their homes or the long term care facilities they live in. Assess patient/family emotional, social and financial needs and problems. Establish goals in the patient care plan to meet the psychosocial and emotional needs of the patient and family. Maintain ongoing support and communication with patients and families in their homes, long term care facilities, the hospital or hospice home. Attend Interdisciplinary Team meetings to discuss the psycho-social and emotional needs of the patient and family and to collaborate with other team members. Explain fee for services and payment mechanisms, including the Hospice Medicare/Medicaid Benefits, for those eligible, to all new patients and families. Complete Financial Assessment when appropriate. Complete appropriate paperwork for admissions, discharges and revocations in a timely manner. Complete appropriate paperwork related to discharge planning, ie FL-2, PASARR, Medicare notices.
