LVN Case Manager: HIV Care & LGBTQ Health Services
Los Angeles LGBT Center
The Los Angeles LGBT Center is seeking an LVN Case Manager within Health Services. Under supervision, you will collaborate with a multidisciplinary team to develop and implement care plans for clients living with HIV/AIDS, coordinating referrals and ensuring adherence to DHSP guidelines. You will maintain patient records, support billing and contract requirements, and participate in ongoing education with a focus on quality assurance and HIPAA compliance. #J-18808-Ljbffr Los Angeles LGBT Center
