Key Responsibilities\n

• Conduct comprehensive assessments to identify the specific needs of each patient, ensuring individualized treatment plans.

• Facilitate clear communication among supervisors, direct care staff, family members, guardians, and relevant stakeholders to track patient progress and adapt care as necessary.
\n
• Develop and submit a tentative schedule of appointments, home visits, and observations to supervisors to efficiently monitor patient care and progression.
\n
• Organize home visits that comply with observational requirements, including chart reviews and staff interviews, to gather data for enhanced patient support.
\n
• Monitor implementation of treatment plans, ensuring that strategies are executed effectively and align with best practices.
\n
• Participate actively in Interdisciplinary Team Meetings and contribute insights on pragmatic treatment solutions.
\n
• Document and complete monthly and quarterly progress notes to maintain accurate records of client advancements.
\n
• Arrange training sessions to certify staff and team members in proper execution of treatment plans and maintenance of documentation according to our standards.
\n
• Provide follow-up interventions via home visits and observations to ensure ongoing support tailored to patient needs.
\n
• Maintain meticulous records to ensure information is accessible to relevant parties, demonstrating commitment to transparency and accountability.

• Act as a liaison between the organization and the community, fostering support for mental health initiatives and development goals.
\n
• Contribute to the creation of training materials aimed at enhancing the quality of our programs and services.

Patient Population / Client Focus\n

As a Licensed Professional Counselor at Oaks Integrated Health Centre, you will engage with a diverse client base, including both adults and youth. Our clients often present with a range of mental health issues such as anxiety, depression, trauma-related conditions, substance misuse, and challenges within family relationships. Your approach will be client-centered, prioritizing collaboration and individualized care to foster effective therapeutic relationships.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.

• Current and unrestricted licensure as a Licensed Professional Counselor (LPC) in the state of Oregon.

• At least two years of relevant counseling experience in a clinical context.

• A strong foundational knowledge of legal and ethical standards in mental healthcare.
\n
• Exceptional interpersonal and communication skills, with the ability to establish rapport with a diverse clientele.

• Proven ability to collaborate effectively within a multidisciplinary healthcare team.

• Proficiency with Electronic Medical Records (EMR) software and related technologies is essential.

Preferred Experience\n
