Registered Nurse (RN) Home Health Shifts Available: PRN Service Areas with Openings: Inner Loop Transition Home Healthcare is an elite home health agency serving Houston, TX and surrounding areas since 2010. Our team is growing and looking for a dedicated: Responsibilities Oversee nursing care team. Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care. Plan, organize, and direct home care services. Conduct assessments of patient care needs and develop care plans. Educate patients and their families on managing their health conditions, medication regimens, and any necessary lifestyle changes. Prepare timely, complete, and accurate clinical notes. Qualifications Valid RN license in the State of Texas or compact license. Home health experience (at least 1 year). Knowledge of home health regulations and general healthcare compliance. Experience with Home Care Home Base (HCHB) EMR (preferred but not required). Excellent communication and people skills are essential. Ability to work independently and as part of a team.
