Job Summary:

Provide specialized physical therapy services to patients with various conditions, supporting their recovery and functional independence through targeted interventions. Collaborate with interdisciplinary teams to develop and implement personalized treatment plans.

Responsibilities:

• 
  Assess patients physical conditions and develop individualized therapy plans based on clinical findings.
  
  
• 
  Administer evidence-based therapeutic interventions to improve mobility, strength, and functional capacity.
  
  
• 
  Document patient progress, treatment approaches, and outcomes accurately in the EMR system.
  
  
• 
  Educate patients and caregivers on proper techniques, injury prevention, and home exercise programs.
  
  
• 
  Comply with all facility protocols, safety guidelines, and licensing requirements.
  
  
• 
  Maintain professional competence through ongoing education and training.
  
  

Qualifications:

• 
  Hold a valid New Hampshire PT license or current Connecticut (Compact) license.
  
  
• 
  Possess active American Heart Association BLS certification.
  
  
• 
  Have at least 2 years of outpatient physical therapy experience.
  
  
• 
  Demonstrate strong clinical assessment and treatment skills.
  
  
• 
  Own reliable transportation for commuting purposes.
  
  
• 
  Maintain current immunizations and state-specific licensure credentials.
