HUD-VASH Social Worker - Veterans Housing Advocate (EDRP)

The Social Worker - HUD-VASH Case Manager at the Bronx Campus provides psychosocial assessment, case management, and treatment planning for Veterans and their families. You will collaborate with the interdisciplinary team to ensure discharge readiness, linkage to VA and community services, and ongoing support to prevent homelessness. Responsibilities include education for residents and staff, ongoing plan reassessments, and facilitating connections to community resources. #J-18808-Ljbffr
