A dedicated Registered Nurse with home health experience is needed for a 13-week full-time contract. This role is ideal for RNs with OASIS expertise, strong clinical judgment, and a commitment to quality care and thorough documentation.

Schedule:

• Full-time, Monday through Friday
• On-call rotation may be required

Key Responsibilities:

• Deliver skilled in-home nursing care for patients with varying needs
• Complete thorough patient assessments and ensure accurate OASIS documentation
• Develop and manage individualized care plans in coordination with physicians and interdisciplinary teams
• Provide wound care, oversee medication administration, and offer patient/family education
• Support disease monitoring and communicate changes in condition promptly to the clinical team
• Travel to patient residences, maintaining professional and compassionate support

Desired Qualifications:

• Active RN license (required)
• At least one year of recent home health experience (required)
• OASIS documentation proficiency (required)
• Experience with HomeCare HomeBase strongly preferred
• Reliable transportation and valid driver's license
• Demonstrated ability to manage a structured, full-time case load
