Job Description

An established healthcare provider seeks an experienced and dedicated Registered Nurse skilled in Home Health to deliver exceptional patient care. This field-based role is integral to supporting patients in the comfort of their homes and promoting positive, patient-centered outcomes. Schedule:

• Monday through Friday, 8:00 AM 5:00 PM
• 13-week contract commitment
• Caseload expectation: 2532 points per week (approx. 35 patient visits daily)
• Daily travel within assigned territory (mileage reimbursement provided)

Key Responsibilities:

• Complete comprehensive OASIS assessments and create personalized care plans.
• Administer skilled nursing care, such as wound and medication management, IV therapy, and chronic disease interventions.
• Monitor patient progress, identify changes in condition, and maintain prompt communication with the care team and physicians.
• Provide patient and family education regarding disease processes, treatment plans, and preventive care.
• Accurately document all care in Home Care Home Base (HCHB).
• Collaborate with interdisciplinary team members to foster coordinated, high-quality care.
• Participate in case conferences, team meetings, and ongoing training.
• Uphold regulatory and best practice standards for patient care and documentation.

Required Qualifications:

• Active RN license in state of practice
• At least 1 year of recent field-based Home Health experience
• Proficiency with HCHB and OASIS
• Reliable transportation, current drivers license, and auto insurance
• Excellent assessment, communication, and organizational skills

Preferred Background:

• BSN degree
• Demonstrated experience managing a 2532 point weekly caseload
• Understanding of Medicare and compliance standards for home health

$1,897 - $2,007/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
