Why This Role Matters As a Primary Care Provider, you’ll be a lifeline for homebound seniors, helping them age safely and comfortably in place. You will provide relationship-based primary care, respond to acute needs, and drive measurable improvements in health outcomes—all with full support from Bloom’s interdisciplinary team and advanced data systems. What You’ll Do Serve as the primary care provider for a defined patient panel, delivering longitudinal, holistic medical care in patient homes and communities Diagnose, manage, and treat chronic and acute conditions with support from our clinical operations, triage team, and MD leadership Address social determinants of health, including housing stability, nutrition, caregiver support, and access barriers Conduct specialty visits such as wound care, post-hospital transitional care, and condition-specific assessments Complete comprehensive evaluations including medication reconciliation, fall risk assessments, advance care planning, and health education for patients and caregivers Utilize Bloom’s integrated data ecosystem to track hospitalizations, coordinate timely post-discharge visits (within 72 hours), and drive superior outcomes Collaborate closely with internal and external care teams to ensure continuity, reduce readmissions, and stabilize high-risk patients What’s In It For You A Manageable, Quality-Focused Schedule Monday - Friday 8-12 patients per day (no unrealistic volume expectations) Light call rotation: only 2–3 times per year Weekends off and flexible daily workflows Robust Benefits – Starting Day 1 100% employer-paid Health, Dental & Vision effective Day #1 3 weeks PTO + 7 paid holidays 401(k) Employer-paid Basic Life & AD&D CME allowance + paid licensing & malpractice Strong support from MDs, triage, schedulers, IT, billing, and marketing—so you can focus on patient care Meaningful Autonomy + Strong Support You practice independently, but never alone.
