Responsibilities
- Deliver coordinated nursing care for an assigned patient load per established standards of care and the nursing process
- Assess data, analyze health data, and develop and implement a care plan to achieve defined outcomes
- Monitor patient status, adjust interventions, and collaborate with the interdisciplinary team to promote safe, quality care
- Supervise and direct duties of nursing staff as assigned; coordinate care with treatment teams and family members
- Provide education to patients and families; participate in discharge planning to ensure continuity of care
- Maintain accurate documentation in the electronic health record (EPIC), communicate changes, and ensure compliance with clinical policies

Requirements
- Current Registered Nurse (RN) license in the state of practice (CA)
- Basic Life Support (BLS) or CPR certification from an approved vendor
- Advanced Cardiovascular Life Support (ACLS) certification required
- EPIC electronic health record experience required
- BSN degree preferred; associate degree in nursing considered with relevant experience
- Minimum of two years of recent nursing experience in the field; acute care experience preferred
- Ability to practice in a home hospice tele-focused setting and to work the night shift
