Key Responsibilities\n

• Conduct comprehensive assessments of patients' psychosocial needs to develop tailored treatment plans and make appropriate referrals.

• Assist patients in navigating the complexities of entitlements, helping them complete necessary forms for supplemental insurance and other benefit programs to reduce reliance on emergency assistance.

• Facilitate access to prescription cost assistance through programs provided by pharmaceutical companies and local community resources.

• Advocate for patients within various community agencies, helping them secure essential services and resources.
\n
• Maintain detailed and accurate patient records in compliance with legal and regulatory requirements while preparing reports and documentation as necessary.

• Coordinate transportation assistance for patients who require it to promote attendance at appointments and access to services.

• Participate actively in clinical meetings, contributing to case discussions and team strategies for improving patient care.

Patient Population / Client Focus\n

You will primarily work with a diverse range of patients dealing with mental health challenges including anxiety, depression, trauma-related disorders, and other psychological issues. Qualifications\n

• A Master’s degree in Social Work, Psychology, Marriage and Family Therapy, or Counseling is required.

• An active and independent licensure, such as Licensed Professional Clinical Counselor (LPCC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT), is essential.

• A minimum of two (2) years of professional experience in mental health counseling or related fields is required.

Compensation and Benefits\n
