Key Responsibilities

• Conduct comprehensive patient assessments, evaluating mental health conditions, psychosocial needs, and strengths to inform a personalized care approach.
• Develop and implement individualized treatment plans that are culturally sensitive and tailored to the specific needs of hospice patients.
• Utilize solution-based interventions and therapeutic techniques to assist clients and their families in navigating complex emotional and psychological challenges.
• Maintain meticulous clinical records and documentation, ensuring compliance with ethical standards while safeguarding client confidentiality.
• Engage in collaborative treatment planning within a multidisciplinary team, working alongside nurses, physicians, and other specialists to ensure holistic patient care.

Qualifications

• A Master’s degree in Social Work from an accredited institution.

Compensation and Benefits
