Work Environment\n

This position offers the flexibility of remote work, allowing you to engage with clients and colleagues without being bound to a physical location. While primarily conducted virtually, you may also need to meet with patients, families, and care teams as needed. Key Responsibilities\n

• Utilize your education, training, and experience to independently assess and formulate psychosocial diagnoses for each patient, employing a compassionate and client-centered approach.

• Partner closely with patients to create personalized treatment plans that address their unique needs and goals, ensuring an interactive and collaborative therapeutic relationship.

• Implement evidence-based psychological interventions and treatment methodologies, including solution-focused therapy, behavioral activation, motivational interviewing, and problem-solving treatment, tailored to the specific challenges faced by hospice patients.

• Work in cooperation with an interdisciplinary team of clinicians, psychologists, and primary care providers to ensure a comprehensive and integrated approach to care, facilitating effective communication and collaboration across various care teams.

• Monitor treatment progress systematically, whether in-person or virtually, tracking responses to interventions and adjusting plans as necessary to meet established goals.

• Support the medication management process, collaborating with medical providers to monitor adherence, manage side effects, and evaluate the effectiveness of psychotropic medications prescribed to patients.

Patient Population / Client Focus\n

This role primarily focuses on supporting patients in hospice care, including those facing terminal illnesses and their families. As an LCSW, you will be responsible for addressing complex psychosocial needs, facilitating grief and loss processing, and providing critical emotional support to navigate the challenges of end-of-life care.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Possession of a current, valid, and unrestricted LCSW license to practice independently in California.

• A minimum of one year’s experience working as an independently licensed therapist in a clinical or healthcare setting, preferably with a focus on hospice or end-of-life care.
