Nurse Practitioner

To enhance patient access and reduce time to treatment, the full-time New Hampshire Licensed Neurology Nurse Practitioner will provide remote clinical care to patients with neurological conditions and collaborate with physicians and the care team. Key responsibilities Provide direct clinical care to patients via telehealth, focusing on neurological conditions and memory disorders Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians Educate patients and families on diagnoses, treatment plans, and disease progression while maintaining accurate clinical documentation Required qualifications Active, unrestricted NP license in New Hampshire; certification in geriatrics, adult-gerontology, or family practice preferred 2+ years of clinical experience in neurology, memory care, and/or dementia care settings Strong clinical judgment and clinical documentation skills Ability to manage a full patient panel independently with physician collaboration Excellent communication skills with patients facing cognitive decline and their caregivers
