Key Responsibilities

• Conduct thorough psychosocial assessments to diagnose mental, emotional, and behavioral disorders, customizing treatment plans based on individual patient needs.
• Implement evidence-based therapeutic interventions through remote sessions, modifying care plans as necessary to reflect the changes in patient status.
• Provide consistent support to individuals and families, offering both individual therapy and family counseling as indicated in the treatment plan.
• Collaborate within a multidisciplinary team, including healthcare providers, community resources, and insurers, to develop comprehensive discharge plans that ensure continuity of care.
• Manage a diverse caseload while prioritizing daily tasks through effective organization and assessment of patient needs.
• Facilitate referrals to additional services such as support groups, psychiatrists, or community programs to enhance patient well-being.
• Maintain compliance with all federal, state, and local regulations as well as organizational policies outlined under the Mental Health Practice Act.
• Participate in care coordination meetings and assist in addressing potential risks such as avoidable delays in treatment or discharge complexities.

Patient Population / Client Focus

The focus of care in this position spans various mental health challenges, including anxiety disorders, depression, trauma, and other emotional disturbances. You will engage with a broad spectrum of clients, including children, adolescents, and adults, creating individualized treatment plans based on their unique situations and backgrounds. Your role as a counselor is instrumental in empowering clients through restorative practices and supportive structures that contribute to their healing journeys.

Qualifications

• A master’s degree in Social Work or a related field, from an accredited institution, is required.
• Current licensure is mandated as either a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC), or Psychologist.
• Strong familiarity with community mental health resources and services to ensure comprehensive support for clients.
• Exceptional written and verbal communication skills, essential for building rapport with clients and collaborating with a multidisciplinary team.
