Pines Village is looking for a motivated and team-oriented Full TIme , RN, MDS Coordinator to join our team. As a member of our care team, the MDS Coordinator will need to work Full TIme in office Monday - Friday and be available for the nursing on call schedule  once a month.

Responsibilities of MDS Coordinator:

• Must have experience with MDS/PDPM process for Sub-Acute and LTC residents
• Must have experience operating Point Click Care for assessments and EMR
• Collaborate with facility’s staff to select the correct assessment of our residents
• Must have good communication skills in order to work effectively and keep accurate, detailed records
• Ensure all MDS are in compliance with State and Federal regulatory requirements
• Must have experience and participate in the Triple Check process
• Participate in developing/updating/reviewing/completion of individualized Care Plans

• Bachelor's (Preferred)

Experience:

• MDS: 1 year minimum (Required)

License/Certification:

• RN (REquired)
• RAC-Certified (Preferred)
