Key Responsibilities\n

As a Licensed Clinical Social Worker at Phoenix Behavioral Hub, your key responsibilities will include:

• Working directly with clients experiencing symptoms of major mental illnesses and severe emotional disorders through individual and family therapy sessions.

• Conducting brief, evidence-based assessments and screenings to determine the immediate and long-term needs of clients.

• Designing and implementing integrated care plans that align with best practices, linking clients to necessary services within our healthcare system.

• Facilitating communication and collaboration between clients, their families, and other healthcare providers to ensure comprehensive care is provided.

• Documenting treatment plans, progress notes, and any incidents in compliance with organizational standards and regulations.

• Tracking and celebrating client progress, empowering them through choice and recovery-focused methodologies.

• Ensuring a safe and therapeutic environment by identifying and addressing any risks or unsafe situations that may arise.

Patient Population / Client Focus\n

Your work will focus on a richly diverse client population, including vulnerable individuals and families facing complex psychosocial challenges. Qualifications\n

To qualify for this role, candidates must possess:
