Work Environment

The role is based in a home care setting, ensuring that our services are accessible to those who need them most. You will have the opportunity to work Monday through Friday with a structured 40-hour workweek. Key Responsibilities

• Conduct thorough evaluations of patients upon referral, developing personalized treatment plans aligned with physician directives.
• Deliver hands-on therapy and rehabilitative techniques in the patients' homes, integrating tools such as therapeutic exercises, mobilization techniques, and patient education.
• Maintain clear communication with patients and their caregivers, providing education on recovery techniques and lifestyle modifications to support ongoing wellness.
• Collaborate with a multidisciplinary team to implement evidence-based practices that enhance the therapeutic experience and maximize patient outcomes.
• Document patient progress meticulously, utilizing electronic health records to track improvements and adjust treatment plans as needed.
• Achieve minimum utilization goals while exceeding patient satisfaction benchmarks through exceptional service.

Patient Population / Client Focus

Your role will encompass a diverse patient population, primarily focused on orthopedic care, including but not limited to post-operative rehabilitation, injury recovery, and musculoskeletal disorders. Additionally, you will engage with pediatric and neurologically impaired patients, providing your expertise across a range of therapeutic specialties. Your ability to establish relationships and trust will be crucial for understanding each patient's unique circumstances and tailoring your approach accordingly.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• A valid State License in Physical Therapy, ensuring compliance with local regulations.

• Basic Life Support (BLS) certification, as provided by the American Heart Association, verifying your readiness for emergency situations.
