Work Environment

This position involves working directly within patients' homes, allowing for personalized interactions and tailored treatment plans that accommodate individual home environments. As part of a supportive team, you will engage in collaborative approaches to patient care, ensuring that treatments are aligned with the latest evidence-based practices. Our onsite work hours are Monday through Friday, encompassing a standard 40-hour work week.

Key Responsibilities

• Conduct thorough evaluations of patients referred by physicians, creating individualized treatment plans based on specific patient needs and progress.
• Implement effective therapeutic interventions, utilizing a range of evidence-based techniques to optimize recovery outcomes.
• Mobilize treatment strategies for a diverse patient population including orthopedic, pediatric, neurologic, and sports medicine cases.
• Deliver care that emphasizes patient education, ensuring that patients and their caregivers understand treatment objectives and home exercises.
• Document all patient interactions and treatment plans meticulously, ensuring compliance with organizational standards and state regulations.
• Maintain open lines of communication with medical professionals to align treatment goals, report progress and adapt strategies as needed.
• Meet organizational productivity standards while demonstrating exceptional customer service and empathy towards patients.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited APTA or CAPTE-approved program.
• A valid State License to practice Physical Therapy in Virginia.
• Basic Life Support Certification from the American Heart Association.
• Strong verbal and written communication skills, compassion, and a patient-centered approach.
• Proficiency in using electronic health record (EHR) systems for managing patient documentation.

Preferred Experience

• Experience in a home care setting is preferred, particularly in orthopedic rehabilitation.
• Familiarity with additional specialties such as pediatrics or sports medicine can be advantageous.
• Proven history of providing evidence-based, patient-focused treatment plans.

Compensation and Benefits

Active Mobility PT offers competitive compensation starting at USD 80,000 to 100,000 yearly, along with a comprehensive benefits package.
