Work Environment

As a member of the True Solutions team, you will work in a supportive, onsite home care setting, providing therapeutic services directly in patients' residences. Key Responsibilities

• Conduct comprehensive examinations and evaluations of patients with functional limitations and disabilities, developing tailored treatment plans based on individual assessments.
• Provide consultation to healthcare providers regarding the patient's continuum of care, ensuring effective communication and coordination of services.
• Deliver high-quality patient interactions by adhering to our customer service standards and fostering trust and respect with individuals of all ages and cultural backgrounds.
• Implement and continuously modify treatment plans based on ongoing evaluations, ensuring they meet applicable regulations and professional standards.
• Maintain accurate and timely documentation for all treatments and services provided, collaborating with other healthcare professionals to enhance patient care.
• Engage in community outreach initiatives, program development, and supervision of support staff, contributing to both department and organizational leadership functions.
• Stay current with best practices in physical therapy by attending staff meetings, workshops, and continuing education programs.
• Ensure effective communication with patients and their families regarding treatment plans and discharge procedures, fostering an environment of support and transparency.
• Monitor compliance with licensure and regulatory standards, maintaining good standing with the state board and all applicable laws.

Patient Population / Client Focus

Your role as a Licensed Physical Therapist will have a direct impact on the lives of a diverse patient population, including individuals recovering from surgery, managing chronic illnesses, or living with disabilities. Qualifications

• Must possess a Bachelor's Degree in Physical Therapy and a valid State License to practice.
• Proven communication skills, both written and verbal, are required, along with proficiency in word processing and electronic health records.
• Current Basic Life Support (BLS) certification is mandatory.

Compensation and Benefits
