Work Environment

The work setting will be primarily in the homes of patients, where you will have the unique opportunity to develop tailored treatment plans in a familiar environment. Key Responsibilities

• Conduct comprehensive evaluations by reviewing patient histories, performing specific assessments, and documenting relevant findings to guide treatment planning.
• Engage patients and their families in understanding the evaluation results and collaboratively setting achievable treatment goals.
• Implement individualized treatment protocols that adapt to the patient's evolving condition and advancements in healthcare methodologies.
• Consult with interdisciplinary healthcare professionals when necessary to coordinate comprehensive care.
• Monitor and document changes in patient conditions to inform adjustments to the care plan or refer for additional medical consultations as required.
• Ensure timely and accurate documentation of all patient care activities, creating a clear record of each patient's rehabilitation journey, including evaluations, progress notes, and discharge summaries.
• Manage time effectively to meet productivity standards while maintaining high-quality patient interactions.
• Participate in marketing initiatives and community outreach programs to promote our services and improve access to care in the local area.
• Contribute to departmental objectives, compliance with regulations, and ongoing quality assurance processes.
• Engage in continuing education activities to promote both personal and organizational professional growth.
• Serve as a clinical instructor to guide and mentor students in practical settings.

Patient Population / Client Focus

Your role as a Physical Therapist will directly impact a diverse patient population, including those recovering from surgeries, injuries, chronic pain, and neurological conditions. Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution.

• American Heart Association's Basic Life Support (BLS) certification for Healthcare Providers.

Preferred Experience

Compensation and Benefits
