Work Environment

This position allows for the flexibility of working in a home care setting, where you will provide treatments directly within clients' homes. Key Responsibilities

• Conduct comprehensive evaluations of clients' physical capabilities, including assessing muscle strength, joint function, and mobility to develop tailored treatment plans that address their specific rehabilitation goals.
• Administer individualized treatment modalities, including therapeutic exercises, traction, massage, and the application of physical agents, ensuring each session is adapted to the client's evolving needs.
• Supervise and mentor Physical Therapy Assistants, ensuring they comply with the established plan of care and provide high-quality support to clients.
• Regularly evaluate and adjust the treatment plan based on client progress and response to therapy, collaborating with interdisciplinary team members as needed.
• Perform detailed assessments of clients' home environments, making recommendations that enhance their independence and safety during recovery.
• Maintain meticulous clinical records including initial evaluations, progress notes, and discharge summaries, ensuring compliance with regulatory standards and submission timelines.
• Engage with clients and their families to educate them on therapy practices and provide guidance for at-home care to maximize therapeutic benefits.
• Participate in case conferences to strategize care approaches and reduce barriers to patient progress, taking an active role in enhancing the overall service delivery.

Patient Population / Client Focus

You will primarily work with a diverse client population, including post-operative patients, individuals with chronic illnesses, and those recovering from injury or surgery. Our focus is on providing respectful, individualized care that supports clients' physical progress and emotional well-being.

Qualifications

• Hold a Doctorate in Physical Therapy (DPT) from an accredited institution.
• A valid and current Physical Therapy license in the state of Virginia, in good standing.
• Current CPR certification is mandatory.
• Demonstrated passion for clinical excellence and a commitment to upholding high standards of patient care.

Compensation and Benefits
