Join Our Caring CommunityHumana's Primary Care Organization is a provider of senior-focused, value-based care, with more than 340 centers across 15 states under the CenterWell and Conviva brands.This is a full-time, on-site, patient-facing role. You report to the Associate Medical Director and are responsible for both in-visit care and ongoing patient management.You will provide comprehensive primary care to senior patients within a value-based care model that emphasizes quality outcomes, preventive medicine, and long-term health management.What You'll DoYou will see 12-15 patients per day, allowing for longer, more in-depth visitsYou will manage patients with complex and chronic conditionsYou will complete accurate, same-day documentation to support patient care and outcomesYou will manage a daily clinical inbox (labs, messages, follow-ups)You will coordinate care with specialists and care team membersYou will apply Medicare risk adjustment principles through accurate condition documentationWhat You Can ExpectYou will work in a value-based model focused on outcomes—not volumeYou will see lower patient volume with higher complexity, allowing more time with patientsYou will collaborate in a team-based care environment supporting whole-person careYou will manage care beyond the visit, including follow-up and coordinationYou will benefit from a supportive culture and work-life balance, including PTO, CME, and minimal callRequired QualificationsMD or DO from an accredited programBoard Certified or Board Eligible in Family Medicine, Internal Medicine, or GeriatricsActive, unrestricted medical license (or ability to obtain) in South CarolinaAdditional Experience We Value2+ years of clinical experience with senior populationsExperience with Medicare Advantage or risk adjustmentExperience supervising Advanced Practice Providers (NPs/PAs)Proficiency with electronic health records (EHR) and digital documentationCompensation and BenefitsYou will receive a compensation package that may include:Sign-on bonus and relocation support (if applicable)Medical, dental, and vision insurancePaid time off (PTO) and CME support401(k) with employer matchMalpractice coverageScheduled Weekly Hours: 40Pay Range: $255,000 - $298,500 per yearThis job is eligible for a bonus incentive plan.
