Dynamic Case Manager: Client Care & Planning Advocate
The Kintock Group, Inc.
The Kintock Group, Inc. is seeking a dedicated Case Manager to provide intake, assessment and ongoing care coordination for its clients in Bridgeton, New Jersey. You will develop care plans, connect clients with community services, educate them on rights and responsibilities, and participate in case conferences and discharge planning.
