Case Manager - Street Outreach & Housing Linkages

The People Concern is seeking a Case Manager (Outreach) to engage vulnerable homeless individuals in Midtown Los Angeles. You will provide case management, connect clients to housing and supportive services, and work within a multidisciplinary E6 team. Responsibilities include aftercare, data entry, and collaboration with social service agencies. A valid CA driver’s license and insurance may be required for client transport. This is a full-time, non-exempt role in a mission-driven organization. #J-18808-Ljbffr SupportFinity™
