POSITION PURPOSE: The Intensive Case Management Services (ICMS) Case Manager will work directly with individuals and families facing homelessness, chronic illness, physical disabilities, or frequent use of the healthcare system. This role is dedicated to helping clients become "housing ready" and then supporting them in achieving and maintaining stable housing. The case manager will also focus on improving clients' physical and mental health, while ensuring they have access to critical resources. Utilizing a person-centered, field-based approach, the case manager will provide holistic support to navigate and coordinate complex healthcare, housing, and social service systems, empowering clients to build a foundation for long-term stability and well-being. - Serve as the primary point of contact for clients throughout their entire participation in the program, ensuring continuous support and follow-up.
- Conduct comprehensive intakes and assessments for new clients to identify needs, eligibility, interests, and goals, and provide ongoing support through monthly home visits to promote housing retention.
- Collaborate with clients and their support networks to develop personalized care plans, focusing on achieving both short-term and long-term goals in areas such as education, employment, housing stability, health outcomes, permanent connections, and overall well-being.
- Work as part of an interdisciplinary healthcare team, utilizing an integrated care model to coordinate services and ensure holistic client support.
- Connect clients to a broad range of supportive services, including behavioral health, healthcare, substance use treatment, Enhanced Care Management, and employment/education resources, by coordinating with external service providers.
- Provide ongoing support, empowerment, education, and case management to clients, ensuring they have the tools they need to succeed and thrive.
- Facilitate access to essential resources, such as Depart of Social Services, Social Security, and Medi-Cal, and serve as a liaison between clients and these services.
- Match clients with appropriate housing opportunities by conducting field searches for options such as short-term crisis housing, Section 8, substance abuse programs, and low-income housing.
- Assist clients in becoming "housing ready" by gathering all necessary documentation and information required for housing applications. Provide guidance in completing housing applications and connect clients to resources that offer financial assistance programs and support with other housing-related paperwork.
- Participate in case conference meetings to coordinate and streamline services, ensuring collaboration with both internal teams and external service providers.
- Document client interactions thoroughly using Nextgen and HMIS.
- Link clients to necessary services and facilitate access to community resources to enhance overall support.
- Advise clients on available healthcare and community resources, assisting them in utilizing these services and following up as needed to ensure successful outcomes.
- Attend regularly-scheduled and impromptu team meetings, maintaining open communication with supervisors and program team members.
- Participate in community resource meetings and training sessions as assigned to stay updated on best practices and community offerings.
- Collaborate effectively with other departments, agencies, and partners to ensure seamless service delivery.
- Actively contribute to care team meetings, sharing insights and updates to ensure comprehensive client care.
- Ensure compliance with HIPAA regulations, maintaining strict patient confidentiality at all times.
- Perform other duties as assigned, contributing to the ongoing success of the program.
- Minimum of one of experience working with homeless individuals and have a social work/mental health-related bachelor's degree or have a minimum of two years of experience providing direct mental health or intensive case management services.
- Have experience working with clients with mental illness, chronic health issues, and substance use disorders.
- Must be comfortable working outdoors with the field medicine team, engaging directly with clients in their environment to provide hands-on support.
- Have expertise in the following areas: chronic homelessness, outreach and engagement strategies, intensive case management services, best practice models, mental health and substance use disorder services, crisis intervention, suicide assessment and prevention, affordable housing and public benefits applications, housing and landlord/tenant rights and eviction prevention.
- Knowledge of community resources in the area of residence.
- Comfortable working with diverse populations including formerly incarcerated individuals, undocumented individuals, LGBT populations, and those experiencing homelessness, substance abuse, and/or mental illness.
- Exceptional ability to connect and engage with people.
- Good oral and written communication skills.
- Detail-oriented, organized and possess time management skills.
- Able to work flexible job hours.
- Willingness to work in various environments, including jail settings, street outreach, home visits, homeless encampments and/or shelters.
- Must possess a valid California driver's license and maintain a driving record that meets EVCHC automobile insurance eligibility standards. This position requires regular travel between clinic locations and community sites.
- Bilingual English/Spanish preferred (read, write, speak).
- Background check, required by County guidelines.
- Minimum of one year of experience working with homeless individuals and holding a bachelor's degree in social work, mental health, or a related field, OR at least two years of experience in providing direct mental health or intensive case management services.
- Experience working with clients who have mental illness, chronic health issues, and substance use disorders.
- Comfortable working outdoors and in diverse settings with the field medicine team, engaging directly with clients in their environment to provide hands-on support.
- Expertise in key areas such as chronic homelessness, outreach and engagement strategies, intensive case management, best practice models, mental health and substance use disorder services, crisis intervention, suicide prevention, affordable housing and public benefits, housing rights, and eviction prevention.

- Strong interpersonal skills, with an exceptional ability to connect, engage, and build trust with clients.
- Excellent communication skills, both oral and written.
- Highly organized, detail-oriented, and proficient in time management.
- Ability to work flexible hours, including evenings and weekends as needed.
- Bilingual in English/Spanish preferred (both written and spoken).
