Work Environment\n

This role is uniquely positioned within a remote setting, allowing you the flexibility to engage with clients and families from the comfort of your home while still being a crucial part of a supportive and collaborative team. Although you will not be physically present at a hospice center, you will maintain strong connections with families through telehealth services and virtual consultations, ensuring that support and care extend beyond traditional settings.

Key Responsibilities\n

• Conduct comprehensive evaluations and assessments to identify the needs of children in early intervention.

• Participate actively in the development and implementation of Individualized Family Service Plans (IFSPs), ensuring a tailored approach based on each child's specific development goals.

• Monitor and evaluate treatment outcomes, collaborating closely with other team members to assess the efficacy of intervention strategies.
\n
• Provide ongoing support, guidance, and resources to families, arming them with the knowledge and skills to foster their child's development.

• Ensure compliance with all federal and state regulations, maintaining the highest professional and ethical standards.

• Intervene proactively with strategies based on evidence-based practices and the principles of the Natural Learning Environment.
\n
• Develop strong rapport with children and their families, creating a safe and nurturing environment conducive to learning.

• Implement coaching models that align with individualized treatment plans to optimize learning outcomes.

• Advocate for each child's needs within the educational and healthcare systems, ensuring they receive appropriate resources and support.

Patient Population / Client Focus\n

This position focuses primarily on infants and toddlers experiencing disabilities or developmental delays. You will be at the forefront of helping these young children, along with their families, navigate the complexities of early intervention services. Your efforts will directly influence key developmental areas, including physical, cognitive, social/emotional, and communication skills.

Qualifications\n

• A Master’s Degree or Doctorate in Social Work from an accredited institution.
\n
• Current licensure as a Licensed Clinical Social Worker (LCSW) by the Department of Public Health.
\n
• A valid driver's license is required to facilitate home visits when necessary.
\n
• Proven ability to adapt to various learning styles to meet diverse children's needs effectively.

• Strong organizational and time management skills to effectively handle multiple cases.
\n
• Excellent verbal and written communication skills to convey information clearly to families and team members.
\n
• Respectful and collaborative approach to working within a multidisciplinary team.
