Overview
Physical Therapist – Home Health, traveling assignment in Templeton, California. This 13-week travel opportunity begins on 07/20/2026 and offers a weekly pay range of $2,096 to $2,304. Work is a day shift on a 5x8 schedule with rotating weekend coverage, in a Short Term Acute Care Hospital setting. This role supports an acute care go-live with Epic (Navigator); no on-call PT license is required. Active California PT license and BLS are required.

Responsibilities
- Conduct PT evaluations and provide evidence-based physical therapy treatment for adult patients in an acute care setting, following physician orders and the plan of care.
- Develop and implement individualized plans of care targeting mobility, gait training, range of motion, strength, balance, and functional outcomes; adjust plans as patient progress dictates.
- Collaborate with the interdisciplinary team, participate in rounds, and coordinate discharge planning and transitions of care.
- Document patient status and progress in the electronic medical record (Epic) and Navigator tools; ensure accurate, timely, and compliant documentation.
- Ensure patient safety and infection control, including proper use of mobility aids and adaptive equipment; comply with hospital policies and core safety standards.
- Provide patient and family education on exercises, home safety, and activity progression; communicate changes in plan of care to patients, families, and team members.

Requirements
- Active California PT license
- BLS (Basic Life Support) certification
- Acute care clinical experience required
- Experience with Epic in acute care settings (Navigator) preferred; no on-call PT license required
- Ability to work day shifts, 8-hour blocks, with rotating weekend coverage
