Critical Requirements

• Must be able to work across inpatient and outpatient settings as assigned.

Position Summary

The Physical Therapist provides evaluations and treatments designed to improve patients' functional abilities across a wide range of ages, diagnoses, and clinical settings. The PT independently develops and modifies plans of care, supervises PT Assistants and designated support staff, participates in interdisciplinary care planning, and provides patient and family education.

The role includes both inpatient and outpatient populations , with a stated minimum of one year of adult outpatient rehabilitation experience.

Key Responsibilities

Evaluation & Treatment

• Perform comprehensive PT evaluations.
• Identify therapy diagnoses and formulate individualized treatment plans and goals.
• Implement, supervise, and modify plans of care based on patient response.
• Provide PT services across various settings and patient populations.
• Review clinical records and case histories.
• Monitor treatment outcomes and provide appropriate recommendations.

Care Coordination

• Participate in care conferences, meetings, and rounds.
• Communicate with referring providers and other healthcare professionals.
• Collaborate with the healthcare team to establish discharge plans.
• Help arrange appropriate durable medical equipment (DME) before discharge.
• Participate in interdisciplinary healthcare partnerships involving patients and families.

Patient & Family Education

• Develop and provide individualized therapy and education programs.
• Create clear home exercise programs.
• Confirm patient/caregiver understanding through feedback and return demonstration.
• Educate patients and families regarding functional limitations, therapeutic exercises, activities, and equipment.
• Promote self-care and independence.

Supervision

• Oversee and direct the work of:
  • Physical Therapist Assistants
  • Support staff
  • Students
  • Interns
  • Other designated personnel

Documentation & Compliance

• Document evaluations, diagnoses, treatments, recommendations, treatment plans, and prognoses according to SEARHC policies.
• Provide services within applicable regulatory requirements.
• Maintain knowledge of Medicare, Medicaid, and third-party billing procedures.
• Follow APTA practice standards.
• Maintain SEARHC confidentiality, quality assurance, and SQM standards.

Equipment & Clinic Operations

• Assist with DME acquisition, measurement, fitting, and delivery.
• Maintain appropriate clinical equipment and supply levels.
• Maintain cleanliness of clinical spaces.
• Assist with clinic operations, administrative activities, research, and specialty programs as appropriate.

Professional Development & Collaboration

• Provide PT-related education to staff.
• Conduct presentations and in-services for medical staff and interdisciplinary personnel.
• Participate in committees and management activities as requested.
• Represent Rehabilitation Services as needed.

Required Education

One of the following pathways is required:

1. Entry-level Master's or Doctorate degree in Physical Therapy from an accredited program, or
2. Graduation from an APTA-approved Physical Therapy program at the master's or bachelor's level plus 2 years of proven work and continuing education experience demonstrating the required advanced knowledge base.

Required Experience

• Minimum 1 year of adult outpatient rehabilitation experience.

Schedule

• 40 hours per week
• Clinic hours:
  • Monday Friday: 7:00 AM 6:00 PM
  • Saturday: 8:00 AM 1:00 PM
• Unpaid lunch: 30 minutes to 1 hour
