Registered Nurse

Primary Responsibilities: Provide skilled nursing care in accordance with the client’s physician orders, individualized plan of care, and applicable DSCC requirements. Assess the client’s physical, cognitive, behavioral, and medical needs and monitor for changes in condition. Administer medications and treatments as prescribed. Monitor vital signs, symptoms, and overall health status. Provide and/or oversee care related to medical equipment and prescribed treatments. Maintain accurate and timely nursing documentation, including assessments, medication administration, treatments, progress notes, and changes in condition. Recognize signs and symptoms of medical deterioration and take appropriate action, including contacting the physician, guardian/caregiver, or emergency services when indicated. Communicate effectively with the client, family/caregiver, physicians, therapists, case managers, and other members of the healthcare team. Follow infection-control, safety, emergency, and agency policies and procedures. Assist with activities of daily living and other supportive care within the scope of nursing practice and the established plan of care. Provide education and support to the client and caregivers regarding medications, treatments, health maintenance, and safety. Maintain client confidentiality and comply with HIPAA and all applicable state and federal regulations. Participate in care-plan reviews and communicate recommendations or concerns regarding the client’s ongoing needs.
