Job Description
Title: Physical Therapist

Schedule: 8hrs

Assignment Details:
- Contract Weeks: 13
- Guaranteed Hours: 40

Requirements:
- Current State License as a Physical Therapist - Required
- Proof of current CPR certification - Required
- Current driver's license. Must have reliable transportation and insurance - Required
- 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required
- Previous home care experience - Preferred
- Bachelor of Science in Physical Therapy from an approved program - Required

Roles & Responsibilities:
- Perform clinical assessments to identify patient needs and appropriateness for care.
- Document and prepare/review clinical record documentation, including Plan of Care, progress notes, and phone orders.
- Plan and provide appropriate treatment for patients with neuromuscular, neurological, and orthopedic disorders.
- Instruct patients and families on transfer techniques and the use of rehabilitation equipment.
- Educate patients/families on therapeutic procedures tailored to their age, condition, and learning readiness.
- Oversee and assist team members in performing patient care consistent with individual patient needs.
- Apply concepts of infection control and universal precautions in patient care activities.

Compliance / Notes:
- Must meet mandatory continuing education requirements.
- Ability to handle heavy lifting, bending, pulling, pushing, and standing for prolonged periods.
