Permanent role Part time (68 hours per fortnight) Social Worker Grade 2, Year 1‑4 (SC21‑SC24) Alfred Emergency/Trauma The Department Alfred Care Group Emergency Department The Role This is a rewarding opportunity to join our Emergency Department and Trauma Unit, working with patients at critical and often life‑changing moments. The position aims to reduce length of stay in the emergency department, prevent admissions to inpatient wards, coordinate hospital and community services, prevent re‑admissions to emergency and improve patient outcomes and consumer satisfaction. The role requires working rotating shifts up to 7 pm. Responsibilities Provide an advanced social work service to patients and their families in trauma services, particularly the Emergency Department, as directed by the Allied Health in ED Team Leader. Conduct a full allied health assessment for complex patients being discharged from the ED. Triage elderly patients while awaiting discharge or admission to an inpatient bed. Perform full psychosocial assessment and initial case management of older patients and patients presenting to the Emergency Department with medical, mental health or social issues not requiring inpatient admission. Evaluate and treat patients in line with professional and departmental standards of practice within a patient‑centred environment. Apply current relevant experience in an Emergency department; beneficial. Understand complex discharge planning for all patients, including older persons accessing TCP/NDIS/VED/HITH/View email address on click.appcast.io and ACAS interface. Shift work – 7:00 am–3:30 pm, 8:30 am–5:00 pm and 10:30 am–7:00 pm. Qualifications and Experience Minimum 3 years post‑qualifying experience in social work, with advanced social work skills and a commitment to ongoing professional development. Demonstrated experience in health care and understanding of diverse patient populations and the issues impacting their lives. Advanced knowledge of hospital admission and discharge planning processes. Ability to work within a fast‑paced acute and outpatient hospital environment and crisis‑responsive community context. Good understanding of adjustment to illness and counseling support. Experience in supervising social work students and a commitment to professional supervision of staff. Ability to develop and lead a portfolio, clinic or program within the team. Strong knowledge of hospital and community settings, electronic medical records and related systems.
