Staff Pharmacist

A direct hire opportunity is available for a Staff Pharmacist at a closed-door Long-Term Care (LTC) pharmacy situated in Mason, OH, a Northeast suburb of Cincinnati. This role focuses on providing clinical pharmacy services within a specialized LTC setting, ensuring safe and effective medication management for patients.

Key Responsibilities:

Clinical Duties (70%)

• Review, verify, and document all new patient medication orders in compliance with organizational policies.
• Collaborate with physicians and healthcare teams to design and implement patient-specific drug therapy plans.
• Conduct ongoing assessments of patient data to identify potential drug therapy problems.
• Set therapeutic objectives and monitor treatment outcomes using laboratory and clinical data.
• Provide drug therapy consultation to patients, families, and healthcare professionals.
• Document all clinical activities and communications thoroughly in patient records.
• Maintain up-to-date pharmacotherapy knowledge through continuous education and training.
• Identify and apply appropriate formulary substitutions and optimize drug therapy according to diagnoses.

Operational Duties (25%)

• Accurately process and verify medications and supplies prior to dispensing.
• Maintain pharmacy records consistent with legal requirements.
• Participate in weekend, holiday, and on-call rotations.
• Assist with orientation and training of new and existing staff.
• Attend scheduled departmental meetings.

Regulatory Duties (5%)

• Dispense pharmaceuticals in accordance with federal, state, and local laws and professional standards.

Qualifications / Desired Experience:

• 1 to 3 years of experience in long-term care pharmacy preferred.
• Familiarity with the Frameworks system is highly preferred but not mandatory.
• Strong clinical decision-making and communication skills.
• Knowledge of regulatory compliance and safe dispensing practices.
