Community Case Manager

We design diverse and innovative programs that provide a high level of structure within communities of care when a person or family is in need. Responsibilities Coordinate services for adults with developmental disabilities at their homes, work, and with community supports. Work collaboratively with families, spouses, providers, and state agencies. Maintain independent, self‑motivated work style and multitask in a fast‑paced environment. Support a very supportive team for assistance and guidance. Qualifications BA in psychology, human services, social work, special education, rehabilitation or a closely related field. Must own a dependable automobile with comprehensive insurance coverage; mileage reimbursement provided. Benefits Comprehensive medical, dental, prescription, and vision coverage.
