Responsibilities
- Provide direct clinical care to memory care residents, including monitoring comfort, mobility, and safety.
- Administer medications and treatments per physician orders; document care in the resident chart.
- Monitor vital signs, notice changes in condition, and report promptly to the supervising nurse.
- Assist residents with activities of daily living (ADLs) and support mobility, hydration, nutrition, and hygiene.
- Collaborate with the interdisciplinary team to implement and update individualized care plans; communicate with families as appropriate.
- Maintain accurate documentation, follow infection control practices, and ensure a safe, respectful environment for residents.

Requirements
- Active Licensed Practical Nurse (LPN) license or ability to obtain licensure for the appropriate jurisdiction
- Graduate of an approved practical nursing program
- Prior experience in long-term care or memory care settings preferred
- Ability to work night shift and maintain reliability and flexibility
- Strong communication, assessment, and documentation skills
- Must be able to pass background check and drug screening
