Licensed Practical Nurse (LPN) - Assisted Living

In an assisted living setting, the Licensed Practical Nurse provides focused nursing support to residents with chronic and acute conditions. The role emphasizes routine nursing tasks, medication administration under supervision, and timely response to health changes.

The core purpose is to support independence while maintaining safety, comfort, and well-being, with documentation in the electronic health record and collaboration with the care team.

Responsibilities

• Assess residents' health status and report changes to the supervising nurse.
• Administer prescribed medications and document administration per policy.
• Monitor vital signs and assist with basic clinical procedures as allowed by state scope of practice.
• Coordinate with RNs, physicians, and facility staff to plan and modify care plans.
• Document care provided in the resident's chart and ensure accuracy.
• Provide wound care or dressing changes under supervision when indicated.
• Educate residents and families on health maintenance and safety measures.

Qualifications

Education: Completion of an approved practical nursing program and eligibility for licensure; relevant clinical experience in long-term care or assisted living is valued.

Licensure: Current LPN license in good standing in the state of practice.

Certifications:

• CPR/BLS certification for healthcare providers
• Medication administration under supervision as allowed by state and facility policy
• First Aid certification or equivalent

Experience or Skills: Strong assessment and documentation skills; ability to collaborate with RNs and physicians; comfort with basic care tasks and using electronic health records.

Experience

• Experience delivering nursing care in an assisted living or long-term care setting.
• Experience documenting in electronic health records and medication administration records.
• Experience coordinating with care teams and responding to health changes.
• Experience with geriatric care and safety monitoring in community-based settings.

Preferred

• Experience with chronic disease management in seniors
• Ability to work in a team-based facility with multidisciplinary staff

Work Environment

Work is performed in an assisted living community or similar residential facility, with a team of nurses, CNAs, and care coordinators. The pace is steady with periodic surges during emergencies or wellness visits; shifts may include days, evenings, or weekends, and documentation is completed in the facility's electronic health record systems.

Residents are adults with a range of complex needs; the role requires attention to safety, privacy, and compassionate communication, while following facility policies and state regulations.
