The Coordinator of Services is responsible for providing instruction and support to their team of staff ensuring the health and safety of the persons served in their home, workplace and within the community. This individual is also responsible for the support, development, and engagement of their assigned team of staff of BAIG. The Coordinator of Services reports to the Director.

This position includes, but is not limited to, the following essential functions:

Essential Responsibilities:

• Communicate monthly with guardians and or family members of individual’s served 
• Lead, as well as provide support and supervision to a team of assigned staff in support of persons served and ensure the implementation of their goal
• Build trust and partnership with employees to drive high performance, engagement, and a strong organizational culture.
• Assist in the referral, intake, and placement process.
• Provide training, oversight, and support to the assigned contracted home provides who are housing with home provide.
• Ensure timely documentation of client interactions and support interventions in the form of progress notes, incident reports, communication log, and other as needed and instructed by direct support staff.
• Write programs for the new client, and safety plans.
• Conduct effective staff and one-on-one employee meetings.
• Work with an ISP team which include State Coordinators, family members and guardians 
• Communicates with the State Service Coordinator to ensure eligibility authorization for funding of services/supports is available.
• Facilities the completion of required assessments, Monthly reviews, home visit.
• Coordinate a meeting with staff weekly.
• Meeting with Director every Friday.
• Ensure adherence to regulations and the contract by having professional and transparent conversations with home provider.
• Assist in the hiring process and coordinate and conduct on-boarding of new staff, as well as be involved in ongoing training opportunities in the form of regular staff and small group team meetings.
• Effectively assign, direct, control, evaluate performance, coach, and hold accountable assigned team.
• Other duties as assigned by Director.

Competencies: 

Knowledge of professional practices in Developmental Disability services.

• Familiarity with health and human services systems including eligibility process. 

• Ability to work independently and as a team member, organize work efficiently, and prioritize response to changing needs of the program, residents, with supervision.

• Ability to problem-solve and advocate effectively. 

• Ability to work a flexible schedule.

• Ability to articulate clearly in both verbal and written communications; 

. Develop and write person-centered ISP Habilitation Programs. 

. Create measurable goals and objectives based on assessed needs. 

. Monitor participant progress through ongoing documentation. 

. Complete timely and accurate Monthly Goal Reviews. 

. Evaluate goal achievement and recommend revisions to interventions and support strategies. 

. Ensure documentation complies with agency policies, Medicaid waiver requirements, and applicable state regulations. 

• Ability to effectively orient and train Staff.

• Proficiency with Microsoft Office Software applications (with emphasis on Word and Outlook).

• Ability to develop productive relationships with clients, families, other professionals, coworkers, and Staff. 

• Ability to interact in a positive manner in person and on phone. 

Education and Experience:

• Bachelor's degree in human services or related field. 

• Two years’ experience working with Developmental disability. 

• Demonstrated knowledge of community resources and services. 

• Ability to work independently, effectively respond to changing priorities, and complete assignments. 

• Basic skills in Microsoft Office applications, including Outlook and Word. 
• Must have reliable transportation and valid Driver's License. 

This position is 40h a week Monday through Friday 08 am- 4pm with 30 minutes lunch time.
