Med/Surg RN

We seek a highly skilled and experienced Registered Nurse specializing in Medical/Surgical care to provide skilled nursing, case management, and patient-centered care during night shifts. The Registered Nurse will perform patient assessments, administer medications, develop treatment plans, and collaborate closely with the interdisciplinary team to ensure high-quality care for individuals experiencing advancing age, frailty, and serious illness. Strong communication and collaboration skills are essential, along with a minimum of two years of relevant nursing experience. Responsibilities:

• Conduct comprehensive patient assessments and monitor patient status.
• Administer medications and treatments as prescribed.
• Develop and implement individualized treatment plans.
• Collaborate with interdisciplinary healthcare teams to coordinate patient care.
• Document patient care activities accurately and timely.
• Provide education and support to patients and their families.
• Manage case documentation and participate in case management activities.
• Ensure compliance with hospital policies, procedures, and safety standards.

• Active Registered Nurse license in the state of Illinois.
• Bachelor's degree in Nursing or related field.
• Minimum of two years experience in hospital, home health, or hospice care.
• Proficiency in clinical assessment, medication administration, and treatment planning.
• Strong interdisciplinary team collaboration skills.
• Excellent communication and interpersonal skills.
• Valid driver's license, car insurance, and automobile for field work.

• Experience working night shifts in a Medical/Surgical unit.
• Additional certifications related to Medical/Surgical nursing or critical care.
• Experience with electronic medical records and documentation systems.

• Position requires working night shifts.
• Ability to work effectively in a fast-paced hospital environment.
• May involve collaboration with hospice and home health care teams.
