Housing Case Manager

Cascadia Health

Cascadia Health is seeking a Case Manager, QMHA, to support clients at Rain Garden and Alberta Plaza sites in Wilsonville and NE Portland. The role focuses on case management, advocacy, and coordinating housing and services to help clients meet treatment goals. You will collaborate with a multidisciplinary team to implement person-centered, harm-reduction and recovery-oriented approaches, while maintaining compliance and documentation across programs. #J-18808-Ljbffr
