Position Summary The Hospice Liaison is responsible for driving census growth by developing and maintaining referral relationships with skilled nursing facilities, assisted living facilities, hospitals, physicians, and other community partners. This role serves as the primary relationship manager between Illuminate Hospice and its referral sources and ensures a strong, ethical, and compliant presence in the community. Territory: Chicagoland Area Essential Duties & Responsibilities: Business Development & Growth Develop and maintain relationships with: Skilled Nursing Facilities (SNFs) Assisted Living Facilities Hospitals Physicians and community providers Generate consistent, qualified hospice referrals Execute weekly and monthly territory plans Conduct in-services, educational sessions, and facility meetings Represent Illuminate Hospice at community events and professional functions Identify new growth opportunities and underperforming accounts Referral & Intake Coordination Serve as first-line contact for referral sources Ensure rapid response to all referral inquiries Coordinate with Intake, Admissions, and Clinical teams Track pending referrals and follow through to resolution Assist with collection of referral documentation as needed Support smooth transition from referral to admission Account Management & Facility Relations Maintain regular in-person facility presence Manage and grow assigned territory and accounts Participate in care conferences and facility meetings as appropriate Address service concerns and escalate issues appropriately Protect and expand existing referral relationships Education & Compliance Educate referral sources on: Hospice eligibility criteria Medicare/Medicaid hospice benefit Illuminate Hospice services Maintain strict compliance with: CMS regulations Anti-Kickback Statute Stark Law HIPAA Company compliance policies Documentation & Reporting Document all: Facility visits Referral activity Contacts and outcomes Maintain CRM or referral tracking system Submit weekly and monthly activity and performance reports Internal Collaboration Work closely with: Intake Admissions Clinical Leadership Administrator COO Attend census, marketing, and operations meetings, as requested Participate in performance improvement initiatives Performance Expectations Meet or exceed monthly admission targets Maintain ethical, compliant marketing practices Demonstrate professional representation of Illuminate Hospice Maintain accurate documentation and reporting Ability to communicate and write professionally Ability to present to large groups and create presentations Qualifications Experience in hospice, home health, healthcare marketing, or case management preferred Strong knowledge of SNF/hospital referral processes preferred Excellent communication and relationship-building skills Organized, self-directed, and results-oriented Computer Experience: Word, PowerPoint, Excel, Outlook Valid driver’s license and reliable transportation Current Automobile Insurance Starting Salary: $85k plus bonus structure (based on experience) Illuminate Hospice values its team members and aims to create a supportive work environment that nurtures each employee’s unique skills and abilities.
